What Families Have Shared
Faridah Mahmud
Shah Alam, Selangor
"After my mother's hip surgery, we weren't sure where to turn. The Transitional Care team at Sereni handled everything from the hospital handover to her physiotherapy schedule. Within five weeks, she was walking confidently again. The staff were patient and professional throughout."
Transitional Care Programme — March 2025
Tan Kim Huat
Petaling Jaya, Selangor
"My father has been a resident for almost two years now. What keeps me reassured is the monthly report — it's detailed and honest. If something is off, they tell us directly rather than glossing over it. That kind of transparency builds real trust."
Comprehensive Residential Care — Ongoing
Rohana Nordin
Subang Jaya, Selangor
"My mother joined the Social Engagement Membership last September after living alone for several years. She was initially reluctant, but by the third week she was telling me about the people she'd met and the mahjong games. I haven't heard her that animated in years."
Social Engagement Membership — September 2024
Ahmad Zulfakar
Ara Damansara, PJ
"The staff remembered small things about my grandfather — that he prefers his tea without sugar, that he doesn't like the television too loud. These aren't things you'd expect a busy care home to notice, but they do. It makes a real difference to how settled he feels."
Comprehensive Residential Care — Since 2023
Lee Wei Ling
Damansara, Kuala Lumpur
"We used the transitional programme after my aunt's stroke. The coordination between Sereni's nurses and her neurologist was genuinely impressive — they kept records, asked the right questions at appointments, and flagged a medication issue that the hospital had overlooked."
Transitional Care Programme — February 2025
Selina Ibrahim
Kelana Jaya, Selangor
"I was nervous about my father moving into residential care — worried he'd feel abandoned. The team took that concern seriously and spent time with both of us before he moved in. He's been there eight months and seems more like himself than he has in years."
Comprehensive Residential Care — August 2024

A Closer Look at Three Journeys
Recovery from Hip Replacement
Challenge
A 74-year-old woman discharged after hip replacement surgery. Her children worked full-time and were not able to provide the consistent daily support she required during recovery.
Our Approach
We enrolled her in the Transitional Care Programme, coordinated directly with her orthopaedic surgeon, managed her pain medication schedule, and arranged twice-weekly physiotherapy sessions on-site.
Outcome
By week four she was mobile with a walking frame. She was discharged from the programme at week five — earlier than the estimated six weeks — and returned home independently.
"I came in feeling uncertain about the future. I left feeling like myself again." — Resident, 74
Rebuilding Connection in Later Life
Challenge
A 69-year-old man living alone in Petaling Jaya following the loss of his wife. His family noticed he was becoming increasingly withdrawn and was spending most days at home without meaningful activity.
Our Approach
He joined the Social Engagement Membership on a monthly trial. Our activities coordinator introduced him gradually to the group, matching him with members who shared his interests in current affairs and gardening.
Outcome
After three months he renewed on a quarterly membership and began attending additional sessions. His family reported a marked improvement in his mood and engagement at home between visits.
"He talks about Sereni like it's a club he belongs to. That's exactly what we were hoping for." — Son, Petaling Jaya
Finding a Long-Term Home with Dementia
Challenge
An 82-year-old woman in the early-to-mid stages of vascular dementia. Her family in Kuala Lumpur were concerned about safety risks at home but wanted a setting that felt warm and personal.
Our Approach
She moved into Comprehensive Residential Care with a care plan tailored to dementia needs — consistent staff assignment, structured daily routines, cognitive engagement activities, and monthly family briefings.
Outcome
Eighteen months on, she remains settled and engaged in daily activities. Her family visits every two weeks and participates in quarterly care plan reviews. They describe the arrangement as the right decision for their whole family.
"The team knows her as a person, not just as a patient. That means everything to us." — Daughter, Kuala Lumpur
