How We Approach Care at Sereni
Every programme at Sereni starts with a thorough assessment of the individual. We look at physical health, daily habits, social preferences, family circumstances, and personal history. This gives us the foundation to design care that actually fits — rather than applying a standard routine to everyone.
Initial Consultation
A conversation with the senior and their family to understand the situation and what kind of support would be most helpful.
Needs Assessment
Our care team reviews health history, daily functioning, social needs, and any relevant medical requirements.
Care Plan Design
A written plan is prepared covering all relevant aspects of support, then reviewed with the family before care begins.
Ongoing Review
Care plans are reviewed monthly and adjusted as the resident's condition or preferences change over time.
Transitional Care Programme
Designed for seniors moving from hospital back toward independent living. Sereni's Transitional Care Programme bridges the critical period after discharge — when medical needs are still active but full hospitalisation is no longer required.
The programme runs from two to six weeks and can be adjusted based on recovery progress. Our nursing team works alongside physiotherapists, dietitians, and the resident's own doctors to ensure continuity from hospital care to everyday life.

Social Engagement Membership
For seniors who live at home but benefit from structured social connection, this membership programme provides a warm community without the commitment of residential care. It addresses the isolation that many independent older adults experience quietly.
Members join weekly group activities at our Ara Damansara facility, including hobby circles, educational talks, guided exercise sessions, shared meals, and occasional excursions. Membership can be monthly or quarterly.

Comprehensive Residential Care
For seniors who would benefit from full-time, professionally supported living, this programme covers all aspects of daily life in a warm residential environment. Private ensuite rooms provide personal space while shared areas foster community.
Families receive monthly written reports and are actively invited to participate in care planning. The goal is a long-term home — not just a facility — where residents feel settled, known, and looked after.

Shared Standards of Care
Privacy & Confidentiality
Health and personal information is managed under Malaysia's PDPA 2010. Access is limited to care staff directly involved in the resident's programme.
Quality Performance Monitoring
Sereni tracks care outcomes, incident reports, and satisfaction feedback to identify areas for improvement and maintain consistent quality.
Responsive Family Support
A care liaison is contactable during working hours for all programme families. Urgent matters outside hours are handled by senior on-duty staff.
Nutritionally Sound Meals
Menus are developed with dietary considerations for older adults. Halal meals are standard, and individual dietary requirements are accommodated.
Safeguarding & Conduct
All staff are background-checked and trained in elder safeguarding procedures. A confidential concerns process is in place for residents and families.
Transparent Agreements
All care arrangements are set out in a written service agreement before any programme begins. Pricing, inclusions, and exit arrangements are clearly stated.
